For Checkpoints and Throughout the Year:
• Create an Evaluation Folder
• Create subfolders in the Evaluation folder using this structure for your documentation (demonstrated in ITS Oct 2011 meeting)

• Please make sure that the entire Evaluation folder is no bigger than 60 MB.
• All student products should be posted on the campus Getting I.T. Right Technology Showcase blog.
• Refer to the Facilitator Eval_12 - Guide.docx in order to know the appropriate folder in which to place your collected artifacts or notes.
• Use the article Technology Experts in Schools: Teacher Leaders or Technicians? as an additional guide for demonstrating ITS efforts.
• Optional: Use the Evaluation-Notes-Template.xlsx as an informal document to record your efforts, artifacts, and progress during each Checkpoint.
• Use documentation only once. We realize that much of the documentation could be used in multiple domains.
• Friday afternoon meeting time during monthly ITS Meetings can be used for collaborating and sharing progress or needs.
• Checkpoint #1 Time Period: BOY - Dec. 9, 2011
• Checkpoint #2 Time Period: Dec. 12, 2011 - March 30, 2012 (was March 9th)
• Checkpoint #3 Final Time Period: April 2, 2012 (was Mar. 19, 2012) - May 16, 2012

End of the Year Evaluation Procedures:
• All evaluations are due to Human Resources on May 18,so we have tried to schedule everyone prior to that date but with as little interference of EOC's as possible.
• Use either the evaluation template provided by HR in May 2012 or the one that we have redesigned. Our "redesign" has-the standard HR information on it and includes a place to add comments/justification/additional information in each domain.

• Save and Name your self evaluation Word document following this pattern:

• Please make sure that the entire folder is no bigger than 60 MB.
• Use documentation only once. We realize that much of the documentation could be used in multiple domains.
• Your principal(s) will be asked to complete an evaluation or to send an email with comments.
• Please complete the self-evaluation form and have it available in your evaluation folder by Wed., May 2. Your eval folder contents will be grabbed on Thursday morning.
• All evaluations will take place at the administration building. Please wait in the lobby for us to come and get you so that you do not walk into anyone else's evaluation.
• Please sign up for a meeting time.
